Introducing my new Internet Consultant service <form-a-day which I believe will serve clients with good value, quick turn-around and a high-quality product. The goal is to create highly customized Web forms or Web applications which are structured such that the client knows the estimated cost by simply reviewing the pricing chart. This is a service, not a product, and the level of customization envelops that fact. <form-a-day will build highly customized forms and install them in a 24 hour period.

Technical Explanation
What is a form? A Web page form is the same as the classic definition of a form where data is requested, filled-out and then submitted. On the Internet, these form submissions are most typically accepted by a database which holds the information. Users of this form can also edit this information by resubmitting the pre-populated form back to the database for updates. These Web forms consist of various inputs called "form elements". Some of these "form elements" come grouped and therefore could be called a "form widget".
An example of a typical form element is a Radio Button set or a drop-down Select Box. The form elements and widgets can be built to serve as input and output for the Web pages. Additionally, other customizations can be built such as auto-responders (automatically generated E-mails), dynamic form elements (upon change of one element this then affects other inputs or layouts), coding logic (example: calculator functionalities within the form itself), exporting data to Excel, integration with the visual design and many many other functionalities all based on the client's needs.Elements vs. Widgets
Some multi-element widgets have a higher premium over and above the "simple" element prices listed and will usually bill-out at one and a half times the typical element price for that element. The list of elements and widgets is extensive and frequently used widgets such as the Dojo Date Drop-down Picker fall into the "simple" element price structure. The Dojo Date Drop-down Picker is a mini-calendar that drops down and allows one to choose a year, a month and the day. Below is an example of a premium widget which lets a customer select from a list of items and then order those selections.

Export data to Excel via .CSV
A very common functionality is the ability to export the gathered data periodically. This functionality comes in the form of a link which may or may not be password-protected depending on your preference, which, once clicked, prompts the Administrator to Save the CSV file to his/her harddrive. This file can then be opened in Microsoft Excel directly or it can also be opened with a text editor after renaming the file to a .txt extension.Web Applications
In essence, a Web Application is comprised of a number of these forms with Add, Edit and Delete capability mixed in with some application logic and associations made across tables in a database. For this reason, it is possible to get a preliminary, first round estimate on the generic base-line structure of an application. This example depicts such a scenario.
